For many years NAPC has led the development of neighbourhood care focusing on population health improvement and integrated, multidisciplinary workforce, as highlighted in Next steps for integrating primary care: Fuller stocktake report, NAPC’s primary care home programme – the precursor to primary care networks, and our wider population health improvement work across the country.

On this page we are sharing some of our learning to date to help colleagues drive neighbourhood care and population health improvement. This includes videos of sessions at NAPC’s spring conference held in May 2022, much of which focused on themes later highlighted in the Fuller stocktake report, including its emphasis on neighbourhood care.
