Practices across a PCN redesigned care by offering multi morbid reviews for patients with long term conditions in one longer appointment in place of multiple single condition reviews. The aims were to reduce the frequency of appointments throughout the year, reduce replication of work thus saving time and money, reduced GP demand, promote self management and improve health outcomes. Results showed 1082 appointments saved by attending multi morbidity reviews verses single condition reviews, 856 hours of GP time saved by using different skill mix and a 44% increase in patients attending by improving the process of contacting patients.
