A population health improvement project which focused on supporting people with long term conditions, developed by a participant on the CARE programme, found that by sending four text messages to people with diabetes over a 6 month period  led to 9 point reduction in their glucose levels. Patients who see this level of improvement see their GP an average of 5 times less per year. The text  messages included offering advice and support to manage their condition. ROI is 2 based on 20 minutes of nursing time to send the text messages saving approx. 40 minutes of GP time. Benefits are also felt by patients and other providers in terms of reduced demand.
