The Challenge

The practice realised there were several patients not accessing screening services or routine health checks. Wayne and Donna then noticed there was a wider gap within their local community meaning that people were struggling to find health and social information to support themselves.​

The Innovation

The digital platform was created so patients could access help and advice they needed. They developed an information hub designed for simplicity, featuring a user-friendly, searchable database covering both local and national resources to encourage connections with communities.​

This initiative has expanded to include social media marketing on platforms like Facebook and Instagram, alongside podcasts aimed at enhancing the uptake of health screenings, providing health and well-being guidance, and directing individuals to appropriate resources.​
