The Challenge
Sam, Chantelle and Jenni increasingly noticed many carers in their practices reported that their health and social needs were not being met. Additionally they found that as caring responsibilities increased, carers often neglected their own health which impacted on their long term health and wellbeing.

The Innovation
The groups project takes a proactive approach, identifying and targeting carers with rising BMI, specific diseases, and anxiety, then extending support to all. Through letters, texts, and follow-up calls, they ensure personalised 1-on-1 sessions, focusing on diet, exercise, structure, and resilience. This also increases carers awareness of the importance of health and wellbeing, creating a ripple effect of well-being within the community.
Health coaching for carers results in a reduction of 5 GP appointments and NHS demand savings of £368 per patient annually.
