Lisa Clift
Clinical Pharmacist & Facilitator
Wrekin PCN & Primary Care Training Hub
The Challenge
Lisa’s practice included many patients taking high risk drugs like anticoagulants, DMARDS, and ACE inhibitors, whose monitoring checks were overdue, posing significant safety risks.
50%
reduction in adverse events possible through improved anticoagulation control
The Innovation
Lisa introduced a streamlined, proactive monitoring approach and worked with administrative staff to manage recalls one month before blood tests expired.
The checks included blood tests, blood pressure checks and weight tracking, all geared to providing more comprehensive care.
Pharmacy technician clinics were also introduced to ensure doses of Direct Oral Anticoagulant (DOAC) medications are monitored.
24%
improvement in timely blood tests and high-risk drug monitoring attributed to the enhanced recall process
The Impact
- Improved safety from clearing the backlog and ensuring timely medication monitoring
- Time saved though a streamlined monitoring and recall process that makes good use of non-GP roles
- Improved prevention from providing more comprehensive care to those who need it
