Why This Programme; Why Now?

Integrated Neighbourhood Teams represent a bold shift in how care is delivered—bringing together health, social care, and community partners to meet local needs holistically. But this shift requires more than structural change; it demands a new kind of leadership.

This programme is built on NAPC’s proven track record of delivering impactful change through working with several innovative areas in the last 2 years on large scale Integrated Neighbourhood Team developments. Building on initiatives we have led including Primary Care Home (PCH) and The CARE Programme.

Programme Structure

The programme runs over 12 months, ideally spanning two financial years to fully embed your enhanced approach to build high-trust relationships, influence across boundaries, and mobilise the multi-agency partnerships.

  • Launch & Orientation
  • Four Core Modules (1–2 days each)
  • Interim Connections (½ day touchpoints)
  • Themes: Relationships & Dilemmas | Culture & Collaboration | Measurement & Empowerment

Two formats are available:

  • Open Programme: For individuals seeking to influence change alongside peers from other systems.
  • System-Level Programme: For leadership teams within the same system/region to drive collective local transformation.
