The CARE Programme – Connected, Authentic, Resilient, Empowered  – is a learning and development programme that empowers primary care professionals to play a key role in their system, to shape services based on population health needs and to strengthen their leadership.

Focussing on wellbeing and building and harnessing resilience, the programme uses digital technology to connect primary care professionals and to support joined-up, multi professional working. CARE’s practical and person-centred approach has already led to more joined-up working in primary care networks, bringing together a range of health and social care professionals as a complete care community

Originally commissioned by NHS England, the CARE programme has been delivered into 41 Integrated Care Systems across the country and is now locally commissioned.

It continues to be delivered to healthcare professionals by the NAPC and is led and delivered by a core team of facilitators and coaches, the programme focuses on:

  • Building personal resilience and wellbeing
  • Improving understanding of population health
  • Developing leadership for system impact

AfterCARE

Support is also available in the form of a tailored ‘AfterCARE’ programme for those systems who wish to continue their learning and development.

Originally focused on general practice nurses, CARE is now available to the wider healthcare team, including pharmacists, physiotherapists, paramedics, practice managers, health and wellbeing coaches, GPs and all roles within community and social care.
