The Challenge
Anna noticed the healthcare system across Nottingham lacked a clear referral process for access to wound care services. The aim of the service was to speed up the healing of wounds and improve outcomes for patients care givers and services.
This approach has rekindled enthusiasm among colleagues and brought back joy into people’s work.
The Innovation
A customised lower limb template was designed to speed up early referrals, clarify pathways, outline available services, and recommend appropriate interventions for healing of wounds.
A revised referral form was introduced to prompt submission of necessary information. In partnership with secondary care, collaborative efforts were undertaken to develop a solution focused on streamlining the referral pathway. Minimising unnecessary steps for patients and healthcare providers.
