Innovation and shaping the future of health policy
Driving and supporting innovation is at the heart of NAPC’s priorities – we are committed to supporting primary care to design and develop innovative solutions and spread good practice.
Integral to this is creating the environment for colleagues from across primary care to come together and have the space to think differently.
Our aims are to:
- support new models of primary care provision and accountable care approaches including the alignment of clinical and financial drivers, to improve patient and population health outcomes and value for money
- improve primary care patient experience and outcomes in a way that matters to patients, by promoting personalised patient care and population health and wellbeing
- develop a primary care workforce that is fulfilled and responsive to the needs of a population by supporting all professions, workforce bodies and providers to improve primary care education and training, empower their workforce, create strong teams and purposeful leaders and
- increase understanding, collaboration and integration across health and social care.
Our work
We have a range of innovation projects and programmes designed to improve patient care and influence the shape of future health and care developments and policy.
In 2012, we established the primary care innovation network (PIN) to support members to become pioneers of primary care. In the first wave, ten practices came together to innovate locally and through this our primary care navigation programme was born. Since then the network have completed a number of projects which include practical interventions that can be spread across the NHS with many focussing on the proactive management of long term conditions.
Our primary care navigation programme provides tailored training for frontline staff to signpost people to local community resources, empowering them to manage their personal needs and reducing their reliance on GPs.
The NAPC’s primary care home programme is an innovative approach to strengthening and redesigning primary care. Developed by the NAPC, the model brings together a range of health and social care professionals as a complete care community to work together to focus on local population needs and provide care closer to patients’ homes.
We have created a platform for local and national primary care leaders across the NHS to shape the future NHS together, enabling them to be at the heart of the healthcare debate. The National Primary Care Network (NPCN) consists of more than 500 professionals drawn from senior management who are driving changes in service delivery.
