The Challenge

Jenny recognised problems with uncontrolled diabetes within her practice. During the COVID-19 pandemic, she observed that uncontrolled diabetes increased the risk of complications. 



The Innovation

Jenny’s innovation focused on improving diabetic patient outcomes through regular text and phone support. While the extra work was initially overwhelming, she realised that using population management data to show positive impact could lead to wider change. By providing this enhanced support, she observed better patient Hba1c levels and increased levels of patient engagement. 



The Impact

  • 34 patients received four personalised text messages each over six months 
  • Messages provided advice and support for diabetes self-management 
  • Text message support led to an HbA1c improvement of 70%
