These six reports from South West London (SWL) evaluate the implementation and context of the Community Health and Wellbeing Workers (CHWW) programme.

This evaluation was conducted to assess the model’s feasibility, acceptability, efficiency, effectiveness, and early impact, gathering formative learning to inform future development.

We acknowledge the leadership and guidance of Dr Mohamed ElShishtawy, Evaluation Lead for SWL CHWW, whose oversight and direction were instrumental throughout the evaluation process. We also recognise Thomas Herweijer, Transformation Programme Manager leading CHWW on the ICB’s side, for his strategic coordination and support in driving the programme’s development across the system.

The success of this work was also made possible by the Primary Care Network (PCN) leads and GP supervisors who provided clinical supervision and critical safety nets for the workers navigating complex caseloads. We recognise the profound dedication of the Community Health and Wellbeing Workers across Croydon, Wandsworth, Sutton, Richmond, Merton, and Kingston. Their ability to establish relational consistency and address the social determinants of health served as the primary mechanism of change in this study. Moreover, we thank the University of Roehampton Ethics Committee for their rigorous oversight and approval of this evaluation.
