Last week, the HSJ Awards celebrated incredible achievements across healthcare, and we extend our heartfelt congratulations to all the winners for their outstanding contributions.
We are also proud to acknowledge two innovative programmes supported by the National Association of Primary Care (NAPC) that were shortlisted for awards:
CHWW Cornwall and Healthcare Central London Pilot
Nominated for the Primary and Community Care Innovation of the Year, this collaboration between NHS Cornwall and Isles of Scilly Integrated Care Board and Healthcare Central London Primary Care Networks exemplifies innovation in addressing health inequalities.
The Community Health and Wellbeing Worker (CHWW) model places workers in areas with the greatest need, supporting 120 households each month. They build trust within communities, listen to what matters most to individuals, and provide tailored support to improve health and wellbeing outcomes.
This initiative highlights how partnerships between the NHS and the voluntary sector can bring real change to communities most in need.
Calderdale Cares Partnership
Shortlisted in two categories—Place-Based Partnership and Integrated Care and Data-Driven Transformation—the Calderdale Cares Partnership has been a driving force in transforming care delivery across the region.
Born from a small initiative by the Upper Calder Valley Primary Care Network in 2020, the programme now spans all five Calderdale PCNs, with support from NHS West Yorkshire Integrated Care Board. Using population health data, the Ageing Well initiative identifies patients at risk of frailty, delivering personalised, proactive support through partnerships between health, social care, and community organisations.
This approach not only improves health outcomes but also eases demand on GP and hospital services, serving as a national example of good practice recognised by NAPC.
