Support is also available in the form of a tailored ‘AfterCARE’ programme for those systems who wish to continue their learning and development.
Originally focused on general practice nurses, CARE is now available to the wider healthcare team, including pharmacists, physiotherapists, paramedics, practice managers, health and wellbeing coaches, GPs and all roles within community and social care.
We recognise the important role frontline staff in primary care can play in signposting to sources of help, advocacy and support, which helps people to feel empowered to take action to support their own health and wellbeing and in turn improves access to primary care services.
NHS England found that effective care navigation could direct over 15% of patients to either teams that could better help them or to the most appropriate healthcare professional for assessment and response – without the need to see a GP.
National care navigation skills training is available for every practice and PCN, offering both foundation and advanced skills training. The aim is to drive transformation and improve patient access at first contact with the practice. Once a staff member has been selected by the practice or network, they can register here.
The national training programme develops key skills such as active listening and emotional intelligence and is making a difference to primary care staff in practices up and down the country. Feedback from participants has been overwhelmingly positive.
