Health promotion faces numerous challenges due to the rise in chronic diseases, lifestyle-related health issues, and health disparities among different populations.
Overcoming these challenges requires multiple approach’s that address the root causes of ill health, promotes healthy behaviours, and ensures access to resources and healthcare services.

The Innovations
CARE has identified 63 projects aimed at:
- Health Creation: Over half (56%) of projects focused on preventative measures by empowering individuals through exercise groups and support networks.
- Patient Activation: Several projects empowered individuals with the knowledge, skills, and confidence to actively manage their health through signposting and local courses.
- Demand Reduction: Many projects are promoting healthier lifestyles and behaviours, empowering individuals to take control of their health, and fostering environments that support well-being.
