Open to current and aspirant primary care home sites
With now more than 200 primary care homes across England, we are staging our next national primary care home event on 1 February. The day-long event will bring the community of practice together to share innovative ideas and best practice around the development of primary care homes.
Lord Warner, former Minister of State for Health, is attending and will speak about the social determinants of health.
After their successful session at the PCH event on 6 December in Manchester, we will welcome back the fire service, who will talk about their innovative partnership working with health services with new case studies.
Attendees can also select from a number of breakout sessions on PCH enablers, depending on their priorities.
Each PCH is invited to bring up to six team members, ideally involving partners to reflect their integrated working. Feedback from previous events has been that bringing a range of roles and partners is valuable, aiding discussion and PCH planning.
As we still have considerable interest in new members joining the community of practice, we are opening the event to aspirant primary care homes and clinical commissioning groups (CCGs) interested in developing PCHs in their area.
