Primary care home national event - Manchester 6 December 2017

Open to current and aspirant primary care home sites

With now more than 200 primary care homes across England, we are staging our first national event open to all our sites.

Speakers include Dr Tracey Vell, GP and Associate Lead for Primary and Community Care from Greater Manchester Health and Social Care Partnership (GMHSCP), Ged Devereux from Greater Manchester Fire and Rescue Service as well as colleagues from Merseyside and Cheshire Fire and Rescue Services.

Andy Burnham, the Mayor of Greater Manchester, is hoping to attend and there will be a recorded address from Lord Warner, former Health Minister, on the social determinants of health.

We also have a number of breakout workshops on PCH enablers – aligning clinical and financial drivers, population health and data analysis, engagement and workforce redesign.

As we have considerable interest in new members joining the community of practice, we are opening the event to aspirant primary care homes and clinical commissioning groups (CCGs) interested in developing PCHs in their area.
